For acute epididymitis most likely caused by sexually-transmitted chlamydia and gonorrhea and enteric organisms (eg, in men who engage in rectal intercourse), CDC recommendations are as follows: Ceftriaxone 250 mg to be given intramuscularly in a single dose plus Levofloxacin 500 mg to be taken orally once a day for 10 days or Ofloxacin 300 mg to be taken orally twice a day for 10 days.
For acute epididymitis most likely caused by enteric organisms (eg, cases that develop after prostate biopsy, vasectomy, and other urinary-tract instrumentation procedures, with sexually transmitted organisms ruled out) CDC recommendations are as follows: Levofloxacin 500 mg to be taken orally once daily for 10 days or Ofloxacin 300 mg to be taken orally twice a day for 10 days.
Antibiotics usually resolve acute epididymitis infections in a very short time, however a minimum of one to two weeks of intramuscularly antibiotic therapy plus two to four weeks of orally antibiotic therapy is recommended to eradicate the offending organism completely. In chronic epididymitis, a four- to six-week course of antibiotics may be prescribed to ensure the complete eradication of any possible bacterial cause, especially the various chlamydiae.
ANTI-INFLAMMATORY DRUGS
Most patients do not need antibiotics and respond to treatment with anti-inflammatory drugs including Aspirin (acetylsalicylic acid),Diflunisal (Dolobid), Ibuprofen, Dexibuprofen, Naproxen. The most effective of these is Ibuprofen, 400 mg to be taken three times daily and continue this for a minimum of 14 days.
